If your child is taking part in this year’s Summer Show and you haven’t completed your participation form yet, you can access the relevant form below.
Please complete the form even if your child will not be performing, so we have everything recorded and don't need to follow up.
If your child attends both drama and dance or singing, please select the Dance & Singing form.
